Dental plans often cover preventive care such as exams and X-rays at 100%, basic work such as fillings at a moderate percentage, and major work such as crowns, bridges and dentures at the lowest percentage. NAIC
Separate dental plans can have waiting periods before they cover some adult services, and you still pay premiums during the wait. HealthCare.gov
Dental HMOs use contracted dentists with set copays. Dental PPOs let you choose from more dentists at a higher cost, usually as a percentage of the bill. NAIC
