The amount you may pay for covered services before the plan begins paying for certain benefits, subject to the plan’s rules.

A fixed amount that may apply to a covered service, such as a visit or prescription. The amount and timing vary by plan.

Coinsurance and out-of-pocket limit

Coinsurance is a percentage of allowed costs you may owe. The out-of-pocket limit caps certain in-network covered spending for the plan year, but premiums and non-covered costs generally do not count.
